On July 5, 1948, England’s national health service (NHS) was formed under the core of principle of providing free care for all at the point of use.

Seventy years on and the organisation has undergone major transformations and is under more pressure than ever before, but is still one of the most treasures institutions in the country.

To mark the NHS’s big birthday, we want to hear from our readers in Suffolk and north Essex about what the organisation means to you, and how you think it can be improved.

The responses to the survey attached will used as part of our coverage of the milestone next month.

If you have an extra special story to tell about the NHS organisations in Suffolk and north Essex, email our health correspondent before July 2.
